The consultation began with an interview of the manager of inpatient behavior health, a nurse leader. Denae discussed that recent staffing ratio guidelines used in the facility. The facility uses a Shift Management Tool (SMT) to staff every four hours and then also uses a Tableau tool verify that the charge nurses are staffing correctly. The facility has implemented a WHPUOS goal. The charge nurses on the floor have been arguing with the nurse leaders and not being fiscally responsible, because they were not held accountable for this before. Denae said that there are some hurt feelings because the house charge nurses are “friends” with the staff nurses and some of the charge nurses are having a hard time putting nurses on call and staffing to the ratios that are laid out. Denae and I discussed the way staffing is communicated to the house charge nurses. All the leaders get together at 1045 and 1500 Monday through Friday and talk through the staffing. Then the leaders were telling the house charge who to put on call. The house charge nurses were putting the numbers into the SMT and it was noted that they were over in hours almost daily. The plan is to develop a better way to communicate to the house charge nurses on who to put on call and when, giving their input based on acuity, and flexing the right people (can a nurse be put on call and a PCA kept on for the remainder of the shift?). Avera Marshall is a critical access hospital that has 15 inpatient/observation beds, 4 ICU beds, and 6 labor and delivery beds. There is also a 10 bed inpatient behavioral health unit. The ED has 6 bays, with one being a trauma bay. There is a 75 bed long term care facility attached to the hospital. There is also a primary care clinic, a specialty clinic with ophthalmology, OB/GYN, orthopedics, podiatry, general surgery, and other outreach services. There is also a surgery center within the facility. The facility is accredited by the Minnesota Department of Health, but is moving toward accreditation by the Healthcare Facilities Accreditation Program with 2018.
