When closing out a project, it is important to formally finalize activities across that project’s various process groups and to officially establish that the project (and all of the work performed under its auspices) is complete. A key component of a project’s closeout is the generation of complete and detailed final documentation for that project—including the compilation of historical information (such as project, files, contracts, and correspondence) and the recording of lessons learned from performing the project—all of which will be stored and retained as part of the project’s knowledge base.

To complete this Team Application Assignment, work with your team to first develop an electronic Team Sign-Off document that each team member will sign (using a different font and color to “virtually” sign off on the project) and date to indicate his or her approval of the team’s Project Closeout documentation. This Team Sign-Off document will be submitted as part of the team’s Project Closeout documentation.
Next, work as a team to combine each of the individually updated Project Schedule documents (which were previously posted to your designated Team Collaboration Area by the team member who updated it during Part V of the Implementing a Health Information Technology Project assignment) into a single, final Project Schedule document file that reflects the complete planning information that was generated by the team for all of the hypothetical project‘s activities and resources during Parts IV and V of the assignment.
Once the single, final file of the Project Schedule document has been generated, the team will work collaboratively to update both the Project Charter document and the final file of the Project Schedule document in advance of closing out the project to reflect any hypothetical risks or scope changes that the team believes could potentially have been encountered during the project’s execution. These updated Project Charter and Project Schedule documents will also be submitted as part of the Project Closeout documentation.
Lastly, each team member will individually generate a 1-page Lessons Learned document for inclusion in the submitted Project Closeout documentation; in the Lessons Learned document—which will become a part of the project’s historical Lessons Learned Knowledge Base—each team member will summarize the learning that he or she personally gained from the process of performing the Health Information Technology Project. The Lessons Learned document should focus on the following areas of consideration, both in terms of the team member’s individual work and the work of the project team as a whole:
What went well?
What did not go well?
What would you have done differently and why?
