Wanted to write a medical report from a consultant and a mri report for an Injury

It’s not assignment style it exactly how the medical consultant and doctors would write it

The patient has been injured since 13th of June on a horse riding incident, fell of the horse then got up and after the session while driving home got into a car accident.
The patient had been of work from June till December, the patient did not break any bones, however suntanned injuries that mri and X-ray could not pick up. Patient had problems walking and with movement.

Can you write three doctors reports from result of mri and X-rays and also a report from a consultant with medical advice to patient. need the diagnosis, the procedure, the anamnesis etc.

Additional information the patient injuries where not life threatening, nor did he break bones, just had very restricted movement please also show progression of injury from the month November
