Assignment #6 Each country’s health care system has its own attributes. This paper looks at the U.S. and Japan healthcare systems. The U.S. healthcare system is unique among developed nations in that not all its citizens are covered by health insurance and some form of universal care (Shi & Singh, 2015). In the U.S., medical care is accessible according to insurance coverage that millions of Americans do not have (Shi & Singh, 2015; CDC, 2017). While there is currently uncertainty regarding the Affordable Care Act (ACA), it was projected even the full implementation of ACA would have failed to provide universal care for all (Parente & Feldman, 2013). Another component of U.S. healthcare is its multiple payer system that has no central agency governing it with a “bottom-up” approach where “each provider and managed care organizations establishes its own fees or premiums” which could lead to rising out-of-control healthcare costs (Altman & Wallack, 1996). On the other hand, Japan has a single payer system or national health care system that has a “top-down control” over total healthcare-related expenditures to keep healthcare spending under control compared to GDP (Shi & Singh, 2015). Given the Japanese government ensuring affordable medical services by imposing a uniform medical fee schedule at the national level for all medical providers and suppliers both private and public (Zhang & Oyama, 2016), Japan’s total health expenditures as a percentage of GDP was 7.6, almost half of America’s at 13.6 percent of GDP (Kuo, 2017). Also, health expenditures per capita in Japan were $1984, less than half of America’s at $4287 (Kuo, 2017).
