Methods: Data analysis methodologies for this study were primarily qualitative due to a small number of completed data sets. 150 students in grades 2-5 were originally identified for participation in the study. 121 sets of parents gave consent and completed the parent checklist developed by the authors. Due to attrition, 48 students completed the questionnaire upon graduation from high school. School records were utilized to triangulate the checklist and questionnaire data.
