Dem onstrate understanding of intensive ( focussed ) care coordination for people with complex , unstable and co mobidity conditions during hospitalisation , including early detection and secondary prevention strategies . The interviewee may remain anonymous . The outcome is t h at the interview provides a motivating , person focussed atmosphere conducive to a conversation about the individuals condition and what it means to live well with this / these condition / s .
